Formatting is something you would have to do it wont fix the cause it
will only fix the problem temporarily. The cause could be many

1. Buggy kernel.
2. Server crashed and fsck somhow didnt run ??
3. Hardware failure.

Buggy kernel would need kernel upgrade. Bypassing fsck shouldnt happen
witohut manual intervention. Hardware failure is best resolved by
changing hardware :).
