Yep. And I still feel iptables is the better tool to do what the OP wants, since it operates a level lower and hence is less resource-demanding.
That too is a factor, but I've found couriertcpd to be quite good at doing this.
This is the most important use of couriertcpd's access file feature. You can allow emails with larger attachments (that your default size limit) from your LAN, allow more SMTP or IMAP sessions from a known NAT box etc.
Yup - comes in handy when you are building a "toaster" type setup and have hardware load balancers all over the place.
