First of all, clamav-milter doesn't quarantine messages, it just bounces them back to the sender. Now this has two issues; firstly, viruses tend to spoof the From address in e-mails, so some poor bugger
you can set this differently iirc (quarantine or trash infected mail)
you can also define exceptions (dont scan mail from this list)
or you could just set something up to strip executable attachments from your incoming mail and ditch clamav
