BTW, mtools supports most of the higher-capacity formatting, and I was able
to transfer data on my 2.1MB floppy (while it lasted) between 2 linux
machines.

I dont know why it happens but it does happen. Maybe the more enlightened
can inform us. I tried every possible way of recovery including running a
magnet over the floppy (and accessing from both DOS and linux).
