Now that the pagefault disabled counter is in place, we can replace the in_atomic() checks by pagefault_disabled() checks.
If a fault happens while in_atomic(), it can only be a bug in the code. Either a pagefault_disable() call is missing or the specific *in_atomic() variants of uaccess should have been used. That's what might_fault() checks when calling might_sleep(). pagefault_disable() is defined in linux/uaccess.h, so let's properly add that include to all relevant files.
