TCP/IP Illustrated Volumes 1 and 2 by Richard Stevens. 
In volume 1 he explains ICMP as a protocol and discusses output of
tcpdump. Very educational.

In volume 2, he disects the BSD networking code. Chapter 11 is a fully
documented source of an ICMP implementation.
