You might check out <http://www.spellcast.com/> who make a PCI adapters that
interface with ISDN PRIs directly (CSU/DSU included in the board). They also
support MVIP bus interface allowing you to switch 64 Kbit/s channels between
timeslots on one or more PRI adapters.

I have no direct experience on these, but will be might be doing so shortly
as a part of a project to build a voice over IP gateway running on Linux.
