First, the SCSI BIOS says:

Start Unit Request Failed

Then, CentOS says:

Buffer I/O error on device sdb, logical block [0-7] (each in turn)

then:

md: pers->run() failed...
mkrootdev: lavel / not found
mount: error 2 mounting ext3
mount: error 2 mounting none
switchroot: mount failed: 22
umount /initrd/dev failed: 2
Kernel panic - not syncing: Attemted to kill init!
