Just a pet peeve of mine ...

See that the kernel has 
* SCSI support
* SCSI tape support
* Verbose SCSI error reporting (kernel size +=12K) 
    (not absolutely needed, but very needed for debugging)
* the right SCSI low-level driver
(as module if you want)

You don't need ftape, you don't have a floppy tape.
So /dev/ftape is right out, as well.  You might want
/dev/(n)st0, though.  (n -- non-rewinding device, which you
probably really want).  /dev/rmt0 might work as well.

There are a couple of things ... afio, cpio, tar, dump/restore,
dd, ... and then there are packages like taper or tob or
... and then there are commerical backup solutions.

Bootable?  If you have a complete HD on tape via dd, just
write it back.  Else create the partitions, put on the data
(perhaps on a partition by partition way) back on the HD.
Then (if you use lilo, as I'll assume here) mount the
partitions (e.g. to /mnt) on the rescue system you are using. 
Then run lilo -r /mnt ...

That should do the job.  Be sure you try it out *first*.
