The logs are of some concern; all the unresolved symbols suggest that
the module shouldn't be being loaded. Are you sure that the kernel
you are running is configured in /usr/src/linux and that at the
time the ftape modules were compiled the same source tree was
available in /usr/src/linux?

You should be using /dev/ftape and /dev/nftape - the permissions for
these are set to root only; you are trying to access the tape from
root, aren't you?
Normally I get a 'device not configured' message when the module
isn't loaded or is not detecting the drive; I'm a bit confused as to
what this message means. What does `mt -f /dev/ftape status` say, and
can you see if anything appears in /var/log/messages by looking at it
before and after?
Try looking down the /proc/ftape hierarchy (I just realized that
having recompiled my own kernel recently, I haven't recompiled the
ftape drivers yet and hence can't see /proc/ftape at the moment).
After attempting to access the drive, see if (I think)
/proc/ftape/0/controller says anything.
