Well, my first advice would be to use a more recent kernel.  Lots of 
bugfixes have been added in the last year.

If that doesn't help, my second advice is to provide more information
about your system: which HC driver are you using, what kind of usb drive
is it, what are the contents of /proc/bus/usb/devices, what happens when
your system crashes, kernel log contents, and so on.
