One of them is addressed by that patch I sent, the other has to do with the way the EHCI controller interacts with the UHCI controllers (the same problem as Oliver is seeing). What shows up in the system log when you do the rmmod/insmod after the resume? It's surprising that things don't quiet down and start working again.
