Well, I'm stumped. I can see no reason why things should work after reloading the driver and not originally. There should be absolutely no difference in the driver's behavior or the state of the hardware. A vaguely similar bug report was posted here: http://bugzilla.kernel.org/show_bug.cgi?id=5777 That one involved an EHCI controller (not UHCI) that wouldn't work until it had be unloaded and reloaded, but maybe the two cases are related. The report includes two different .config files; one of them would fail like yours and the other would work! So far no one's been able to figure out which part of the configuration was responsible for this. Perhaps if you try both configurations you'll also find that one of them works as it should.
