In reality both logs show a failure occurring during a write.

There's no obvious reason why the device should crash like this.  
Apparently it just doesn't like 120 KB writes.  You wouldn't think
something like that would bother a flash device, but in this case it does.

Perhaps if you set max_sectors to 64 it will help.  (See the explanation 
at <http://www.linux-usb.org/FAQ.html#i5>.)
