Yes.  I have even seen one -- but I don't have one.

No way to know.  When there's not enough power available, almost anything 
can happen.

I don't know, because I didn't write the driver.  Maybe you should ask the 
person who did write it.  My guess is that the driver wouldn't send new 
firmware to the device if it wasn't necessary.
