I'm experiencing regular USB keyboard/mouse lockups on my nForce 430
(x86_64) based Dell E521 using 2.6.20-rc1 (also on 2.6.19, so this is
not a regression). The 430 is a problematic chipset in general it seems
(as is the crappy BIOS in the Dell), and 2.6.19 was the first kernel to
boot without the need for 'noapic'.

Basic scenario is keyboard or mouse stops responding after 10-15 minutes
of use. Note that "use" is important...I can let it sit unused overnight
without a problem, but interactive use of either input device results in
a lockup fairly quickly. Also of note is that only one device locks up
at a time. For example, if the mouse locks up, the keyboard will
continue to work and vice-versa.

Unplugging the device and replugging results in...

usb 1-5: USB disconnect, address 2
ohci_hcd 0000:00:0b.0: IRQ INTR_SF lossage
ohci_hcd 0000:00:0b.0: leak ed ffff8100016660a0 (#82) state 0 (has tds)

...after which no further USB activity is seen. The device is no longer
recognized and all USB devices are dead.

Mouse and keyboard are emulated by a KVM which works reliably for me on
2.6.19/2.6.20-rc1 on non-nForce 430 machines.

USB/input-related dmesg below. I'm happy to provide more info, turn on
debug options, try patches, etc.
