This looks like it might be a problem with the usb-ohci driver, not with usb-storage.

The usb-ohci driver from 2.4.29 needs wmb() inserted in at least one place.
I have a very old version of the SOC vendor's patched ohci driver and got the idea from there to add wmb() in several places. This solves my problem. I am certain that I have put wmb() in unnecessary places, but at least it is working now.
