Okay.  Each was for a single UHCI controller, and each controller
apparently had 2 ports.  The files indicated that the first controller
wasn't working and the second was.  Although it wasn't doing
anything since there didn't seem to be any devices plugged into it.

You mean the port on the _card_ doesn't seem to matter?  How many USB 
ports actually are on that card?

Then either the card, the cable connection, or the hub is broken.  If the 
hub works okay in other circumstances, it's a good bet that the card is 
broken.
