I don't know of any documents describing the format of a stack trace.  If
you're familiar with the kernel source code, the meaning is pretty
obvious.  The trace lists, for each process, all the values on the 
process's kernel stack that might be pointers to code.  It also indicates 
how far the address is from the beginning of the function it's in.
