I can't remember the way to reproduce this right now... the drugs are
getting in the way... however, I can say that I unlink the URB by calling
usb_release_irq() when I get the disconnect callback.
There are basically 3 possibilities for reproducing:
(1) Load all drivers, attach a device, and remove the device
(2) Load all drivers, attach a device, then remove the HCD
(3) Load all drivers, attach a device, then remove usb-storage
This presumes that you have a CBI device (such as a Y-E Data floppy
drive). You don't have to access the device to see this behavior. The
latest patch I sent to the list includes debugging output to show what's
happening and all the relevant values in dmesg
