I have not tried this host controller on Linux yet. However, from my
experience at work (driver writing under windows), I have had bad
experiences with the CMD cards, especially the 670. They seem to be very
broken; CMD used to have a patch (filter driver) to make the controller
behave so-so under Win98; it doesn't work at all under Win98SE.

We were having a number of problems with these cards and our devices; in
trying figure out the importance of fixing the issues, we found out that
these boards have never really been in mass market; they have been sent out
as evaluation boards, and apparently these chips are built into embedded
systems. Where did you get yours from?

If you have any other board lying around, you might give that one a try.
Maybe there is something that can be done in usb-ohci, but I'll leave that
judgement up to the ohci gurus ;-)
