Thanks. This confirms the reason for the problem. It's a hardware bug in the OHCI controller; after completing a transfer, the controller didn't add the Transfer Descriptor to the Done List. I'm working on a patch to detect this sort of mistake and recover from it. I'll let you know when it's ready for testing.
