There are other settings you need to play around also..those are
1. PnP OS = yes/NO
2. Reset Configuration data = ENABLED/disabled
3. resources controlled by = auto/MANUAL

try the CAPSed settings first and then see if the card is being detected.
otherwise try combinations from above and each time check if the card is
being detected.
And by chance, do you have additional pci card, or for that matter another
realtek pci card, in the same machine?

Also the realtek website provides a diagnostic utility for the card. It is
supposed to be run from safe mode command prompt (yeah, M$). If the utility
could find and diagnose the card well, the card should be detected in Linux,
or try noting down the configuration info (irq/io) given by this util and
pass them to kernel directly.

also make sure that you are using "8139too" module
