Weird.  That is really, truly broken.

You didn't mention the FPGA/IP core you are using.  I'm willing to
bet the bug is in there.

[ No need to embarrass anyone.  So many buggy IP Cores in this world.
  It could make someone cry.  But not me, I'm used to it by now :( ]
