I use VoIP and my cellphone exclusively, including rolling my own, and Vonage.  
No landlines here at all.

I don't know anything about using VoIP for text messages, however.

Outside the US the +1 is the country code; here in the U.S. cellphones always 
work with or without it.  When you call the U.S. from your iPhone who is the 
carrier, or carriers?

Numbers for many countries are not easy to get.  Even in major first-world 
countries; it took me quite some time to find a provider for UK "local" 
numbers, for example.

The problem is that many countries don't like interconnect with VoIP, others 
for example in the US) require that you get a number from a company with a 
presence in the area (CLECs in the US).

There may actually be government requirements, prohibitions, etc.

Should be easy.  Even vonage allows ringing up to five numbers at the same 
time (not sequential), and then whichever you pick up gets the call.  They can 
be foreign or US numbers, but you pay for calls to many countries even on the 
best Vonage plans, and they're not cheap by today's standards.

Hard to impossible.  If Skype doesn't do it, then it's likely that it's 
anywhere between hard and impossible.

I believe you can do that with Asterisk implementations (we're currently 
playing with PIAF (PBX In A Flassh), but I'm not sure of the details; numbers 
are very cheap and we have a separate fax number.  Actually we get our number 
from SmartFax; they're the same company as eFax, but much cheaper, and they 
forward an image to my desktop/laptop.  $6.95/month if I recall.

Find a source for numbers first.  Otherwise the rest isn't really going to 
help you <frown>.

we pay significantly less than they charge; their pricing is good but not 
cheap.

If you need a UK number that's local anywhere, I got mine from aaisp.net.

(They're a special kind of number in the UK)

Perhaps more later, as others write.  I'm doing a conference call with on of 
my VoIP vendors today; hopefully I'll be able to pick their brains.
