Hello,
        If there is an entry in fstab it must auto ON while reboot. Why there 
is double entry of SWAP in fstab, one commented, try keeping one entry. what is 
the output of "# fdisk -l", check for proper file-system in fdisk -l that your 
swap shows /dev/sda2. Also check e2label on /dev/sda2, it must give bad magic 
number.......!  Show your full fstab.
 
Check # cat /proc/swaps

I am using fedora core 6 with kernel 2.6.22.14-72. fc6. and I am using Apache, 
mysql, php applications.
when it reboots swap gets disabled and i have to manually on it.
fstab entry is
#LABEL=SWAP- sda2 swap swap defaults 0 0
/dev/sda2 swap swap defaults 0 0

Hi,
Your swap is already ON as it is showing in # free, you dont need to ON the 
swap again or create swap with mkswap command.
Can you tell which Linux you are using and the Kernel version?? Application 
used?

Once this stops rebooting your system, then do the following.

mkswap /dev/'swap partition'
swapon -vagedit /etc/fstab
UUID='your correct swap UUID' none swap sw 0 0

replace texts in single quote with actuals.
when you edit your /etc/fstab file make sure that your swap partition has
the new UUID.
something sort of that
see if that could help
