I don't know about any policy, nor have I used get_maintainer.pl... I just send my patches to the relevant distribution list(s) and CC whoever I feel warrants specific notification. I figure that those who would be interested in reviewing or merging my patches are already subscribed, so there's little point in CCing somebody unless I want to be sure the patch shows up in their inbox.
