I don't know about any policy, nor have I used get_maintainer.pl... I
just send my patches to the relevant distribution list(s) and CC
whoever I feel warrants specific notification. I figure that those who
would be interested in reviewing or merging my patches are already
subscribed, so there's little point in CCing somebody unless I want to
be sure the patch shows up in their inbox.

My process could be bad in all kinds of ways, but it seems to work out
most of the time :D
