It turned out the problem was that I had my Device entry wrong (I needed /dev/input/event4, not /dev/input/event0), so the Wacom driver wasn't actually being used.

However, I was running gpm, which somehow treating it as a USB mouse and repeating it on /dev/gpmdata, together with my ordinary mouse, so it looked like it was working. How it was reading a USB mouse from /dev/psaux I don't know...
