Shouldn't be anything you have to do in the preferences--that's really weird. First thing I'd try is making sure DW is all patched up (they had an updater not terribly long ago) then try uninstalling and reinstalling the extension. All I did to install mine was double-click the mxp file and it seems to have worked. I'm on DW MX 2004 and XP Pro (worked fine on the Mac as well).
