How do you apply the logic of that? Oxford dictionar-il-y speaking a form is simply "a printed document with blank spaces for information to be inserted" and I could think of many instances where both requirements fit eg if you ask the age of the applicant for a friends network it could be...

a. because the business wants to filter out minors > required for legal reasons
b. to plot the site demographic > required for business reasons
c. to help in matching age groups > required for a more productive use of the site.
etc...

Out of context that quote just seems a little philosophical.

Not that I'm picking on you of course! ...on the contrary I can do nothing but thank someone whose work for ABC Kids Online provides tired parents a half hour respite here and there ;-)

Of course there is always the question: if the fields are optional, why include >them at all?

One of the most interesting things at the Nielsen/Norman thing a few years back was a session on forms. One sentence I've never forgotten is "Is your form really a form, or is it a questionnaire?".

A form is something you *must* fill out to get a result, as in, withdrawing money from your bank.

A questionnaire is just one party asking another for information. "Where did you hear about [ProductName]? TV/Radio/Word of mouth/Other"
