It is called a "LED Desk Lamp"

Brand Name "Portfolio"

The lamp has a flat clear plastic cover over the LEDs that has a
transparent rim around it.  I found the lighted-up rim to be a distraction.
It is held in by two screws.  I removed the screws, removed the lens, and
replaced the screws with a nut under the head of each screw to act as a
spacer to hold the LED PCB up into the lamp and I like it even better now. 
I might paint the rim of the lens black and replace it if the proper
round-tuit appears.
