That was part of the other poster's example, it's not a part of the
log4j framework.  You have to associate the trigger evaluator with the
appender, most people do it via configuration file, programmatically you
would do it via SMTPAppender#setEvaluatorClass.

Sure.  In a servlet:
MDC.put("foo", request.getParameter("paramName"));
LOGGER.info("some message");
