On 21-Jun-2002 I emailed a completed LSB Certification Pilot
Questionnaire to [EMAIL PROTECTED] per the
instructions in the questionnaire. However, this address does
not appear to work. See the failure message below. Can you
please advise me how/where to send the questionnaire?
