For starters, whats your video card? If it is an older ATI Mach64 card, use
this in your lts.conf: 

XSERVER = XF86_Mach64 

I have done this successfully. It still might not work unless you get the
right modeline specified...
I had to do: 

X_MODE_0           =  800x600 50.4 800 840 968 1056 600 601 605 628 +hsync
+vsync

To get mine to work at 800x600 in 16bit.
