Last I checked Technology Associates were talking to Bank of Baroda about implementing a video conferencing solution. The one off was about $8000 for hardware then the rest would be bandwidth costs. I think it would be a worthy investment for an ISP to provide such a service. I think MTN also has an in-house adhoc solution it has offered to select customers I forget which. Mainly NGOs.
