The following fixes are included according to Alex's inspection:
-- as ldlm_cli_enqueue does not always create request,
ldlm_resource_get_unused() is added to callers which does it
(mdc_enqueue, osc_enqueue);
-- missed ldlm_lock_reprocess and dput are added;
-- cleanups.
