University Micorfilms International (UMI) is now called Pro-Quest, but they retain Dissertation Abstracts and the UMI initials. I tried my best to find Kevin's dissertation using DA, and it seems not to be there. It could be that since it is published, the dissertation version has been withdrawn.
Prices are about $45 for paperbound print of a dissertation (if available). Available dissertations can also be consulted on-line and downloaded for free if you have library access to Dissertation Abstracts!!
