That's probably the X screen saver.  See xset(1).

Can't help there.  Try Ctrl+Alt+F1 and back; try sshing in.

Surely closing lid triggers a completely different path, e.g. suspend-to-RAM?

You could restart just X instead.
You could look at logs and dmesg and see if any processes are hung.
You could even try triggering a suspend-to-RAM and see if the video BIOS
tweaks *that* does on restore, are enough to unbugger the video.
