Alex, You could use the SNAT mode i.e. proxy: http://blog.loadbalancer.org/enabling-snat-in-lvs-xt_ipvs-and-iptables/
However if you are going to do that it would make far more sense to use HAProxy http://haproxy.1wt.eu/ Can't you even change the default gateway?
