Inset and include don't have forms in the form/ directory thats why they
were missed -- insetbib however I'm sure should have been fixed...Hmmm.

I have also noticed this.  Seems to be a little more work needed with
"click-on-an-inset-button" handling otherwise Asgers mouse changes are
quite excellent.
