1) What do they do then to write Hebrew inside a math 
   formula?
   I guess they can use this sequence of commands: 
   "\mbox{\beginR{} RTL \endR}". At least it is possible
   to do so with ARABI package for Arabic.

2) With ARABI package there is also one other way to write
   Arabic inside a math formula. Currently the commands 
   "\ARmbox{RTL}" and "\FRmbox{RTL}" are used to write 
   Arabic and Farsi respectively. However I guess it is
   fairly easy to adjust the "\mbox{}" command itself
   to do the correct thing automatically.
