The class is "article (AMS)", also "Theorems (AMS-Extended)" module.
After further investigation I narrowed it down to a simple testfile; see
attachments, included are:
test.lyx (source), test.tex (pdflatex export), test.pdf (resulting
pdf with Theorem env in italic).
All the contents and all the modules are needed (i.e. removing any
of them also removes the italics). It seems that entering "Notation*"
env is somehow important.

It is a bug, and your report was very helpful in pinning it down.
I have a zip archive containing some
fixed modules, along with a few new ones (numbering theorems, lemmas
etc. separately by species) at no extra charge (because I'm too lazy to
sort them out). I suggest you install in your local layouts folder
rather than overwriting the official ones. Then reconfigure and restart
LyX and you should be good to go.
Unfortunately, I access the list through GMANE, and periodically either
GMANE or the list server (I'm not sure which) decides I am not allowed
to attach zip archives, even small ones (such as this). Then again,
sometimes it does let me use attachments. The gods are capricious, and
today apparently not in a good mood (since my previous attempt to reply
bounced due to the attachment). Ping me at rubin AT msu DOT edu and
I'll mail it to you directly. I've also entered a bug report (and
proposed fix) in the TRAC system.
