It's quite easy, I've done it.

I leave it to you to use the markup (:oswatersched lyx:) in some
suitable way on the download page. To me it seems it a little bit
wide, and I'm not sure how easy it is to understand what the icons
mean.

The general principle I use for embedding custom HTML is to create a
custom wiki directive, which invokes a PHP function. That function can
then inject whatever HTML we like. If you want to look at the
.php-code, it's in <www-user>/farm/cookbook/LyX/oswatersched_LyX.php
