In order to use a LaTeX class in LyX, you have to have a corresponding
layout file (which essentially maps elements of the LaTeX class to
visual elements in the GUI). As far as I know, LyX does not provide a
layout file for the exam class. There is a user-provided layout at
https://github.com/fuhrmanator/lyx-layouts. Assuming you have the TeX
class (exam.cls) installed, you can download the layout file from
GitHub, put in your local layouts file (~/.lyx/layouts), reconfigure and
restart LyX and try that.
