Your version of xforms has been compiled for glibc2.0 (libc6), whereas
your system uses glibc2.[12] (libc6.[12]). Please upgrade to a proper
one from
ftp://ftp.lyx.org/pub/contrib/xforms-0.88-3.i386.rpm
