Indeed. I just fixed it.

'M-x math-insert \cases' should work now. No easy way to start with an

Hm.. the problem here is that there is a plain tex \cases. But LyX would
barf on that anyway....

I'd really like to make "Use AMS" on by default and drop all AMS auto
"detection" as this fails too often.

It does. But it is sort of mathed feature nonetheless. You can get some of
the \begin{foo}...\end{foo} environment by typing \foo. \array and
\subarray etc should work as well.
