LyX 1.4.3 is quite old. The documentation has been improved a lot in the meantime. When you find bugs in the documentation that comes with LyX 1.5.6, please report them to this list.
*1. User defined math macro section 5.6.
This feature has been completely reimplemented for Lyx 1.6.0 that will be released soon. I'll therefore rewrite these sections soon.
