  My advice: ignore the toc with docbook. The dsssl style sheets that control
the different outputs are ignoring them! They have special parameters where
to put the toc, to hide it...

  The problem you have is that according to the docbook dtd the toc should go
in a very specific place.

  So the simple option is: don't use it, if you want you will still have it in the 
final
output...
 
  If ignoring ;-) my advice you want to put it I can tell you to do it...

  The only code that changed with 1.0.4 was the addition of docbook as far
sgml related stuff is concerned...

  With your sugestion and doubts we can create a better template to help you
and others...
