Steve, Thank you very much for the suggestions, but I think (since filing the brief on lined printed paper in the copier) I have just about got it to where lyx will print the lined and numbered paper, along with TOC, index list and indexes. If you open the attached file in lyx 2.2.2 and print it with Cntrl-D, and then ignore the many errors with "show output anyway", you can see I am almost there. (I deleted most of the brief to avoid boring you). John PS Computer Troubleshooters looks very interesting.
