Check and see if there's a LaTeX documentclass appropriate to your university. If not,

Failing that, use ERT to \renecommand the \maketitle command to meet the requirements.

Usually the best thing to do is to build a box of the dimensions required by all of the elements, fill it and place that in the appropriate place on the page.

A search on ``renewcommand maketitle'' should get you started.
